The applicant must have obtained the equivalent of nine (9) months of full-time professional experience (the Supervised Professional Employment) in which bona fide clinical work has been accomplished in the major professional area (speech-language pathology or audiology) in which the license is being sought.

The applicant must have passed a National Examination in Speech-Language Pathology and Audiology, either the National Examination in the National Examination in Audiology. Each year at its first meeting, the Advisory Council will recommend to the State Board of Health a passing score for the coming year.

The applicant must have completed a clinical practicum in marriage and family therapy of no less than 12 consecutive months that provided him or her face to face clinical practice with individuals, couples and families and allowed the applicant to apply classroom learning to actual marriage and family therapy practice situations in the field setting providing for live supervision. The practicum student must have completed a minimum of 500 face to face clinical client contact hours of which no less than 200 must be relational contact hours with couples and/or families and must have received supervision with a ratio of no less than one (1) hour of supervision for each six (6) hours of clinical client contact. The practicum experience must be supervised by an AAMFT approved supervisor or Supervisor Candidate under supervision-of-supervision by an AAMFT Approved Supervisor.

Additional requirements specific for schools? | 5-year Educator License: Requirements for a Bachelor's Level License for Speech/Language Therapist (216): The 216 license is a five-year renewable standard license. The State Board approved licensure guidelines for this license state that the 216 license holder will be limited in the type of services that will be provided to students. The 216 license holder can provide articulation therapy, articulation testing, write IEP's and provide speech therapy. The 216 license holder can neither provide language therapy, language assessments, voice or fluency, or corresponding assessments, nor chair an eligibility determination committee. All 216 license holders must be supervised by a 215 master's level certified Speech-Language Pathologist (SLP) in each school district. Candidates holding a Bachelor's degree in Communicative Disorders, Speech Pathology, or Speech and Hearing will be able to file a license application to the Office of Educator Licensure for the 216 Bachelor's level license for Speech/Language Therapist on or after July 1, 2013. Candidates must submit an official sealed or electronic (eScrip) transcript verifying the degree. In addition, the following requirements must be met for the license. 1. Those that have held an Interim Certificate (IC) emergency license 215 in Speech-Language Pathology (SLP) must have: - passing scores on all parts of Praxis I - Reading, Writing and Math - certificate of completion from 216 Summer Institute (four required sessions); or purchased CDs and passed the required competency test - signed documentation verification form showing 100 hours of practicum experience in articulation therapy* or proof of progress toward meeting TMI certification requirements. 2. Those that have held a 1-year 215 Emergency Certificate (EC) in SLP who are certified teachers in another area and teaching out of field must have: - certificate of completion from 216 Summer Institute (four required sessions); or purchased CDs and passed the required competency test - signed documentation verification form showing 100 hours of practicum experience in articulation therapy* or proof of progress toward meeting TMI certification requirements. 3. Those that have graduated from a Bachelor's degree SLP program without practicum experience and have not held any license must have: - passing scores on all parts of Praxis I - Reading, Writing and Mathematics - certificate of completion from 216 Summer Institute (four required sessions); or purchased training CDs and passed the required competency test |
